Thanks for being a part of our ongoing effort to bring important information to the blind, visually impaired and the general public through our user friendly streaming audio / video programming.
PLEASE NOTE THAT THESE FORMS MUST BE SUBMITTED NO LATER THAN 10 DAYS BEFORE SHOW DATE
(Failure to do so will postpone your interview)
There are two steps to complete this process. The FIRST STEP is to complete our AUDIO / VIDEO RELEASE FORM.
Click the link RELEASE / GUEST FORM below (between the blinking arrows) and complete the FIRST form. When you click the SUBMIT BUTTON your browser will take you to STEP #2 which is our GUEST FORM. Enter your information in the form provided. PLEASE try to include all information requested when applicable. PLEASE COMPLETE BOTH FORMS IN SUCCESSION! Failing to do so will postpone your interview. Once you complete the second form click the SUBMIT BUTTON on the bottom of the page. You will then be taken to the last page where we will confirm receipt of your submitted forms and supply you with additional information.
